Urge Congress to Authorize Vital Rural Health Programs

Several key rural health programs at the Federal Office of Rural Health Policy (FORHP) need to be authorized in statute or are up for reauthorization.  

“Authorization” and “reauthorization” mean that programs need language in a statute in order to be established or continued. Generally, Congress must pass legislation to establish or continue programs, agencies, offices, or projects, and then Congress can fund these programs through the appropriations process. The following programs are of utmost importance for Congress to reauthorize in 2025 to in order to support rural communities’ access to care and sustain the rural health infrastructure, workforce, and services: 

  • Medicare Rural Hospital Flexibility (Flex) Grant Program  
    • H.R. 3684, the Save America’s Rural Hospitals Act, reauthorizes the Flex program to help small rural Critical Access Hospitals (CAHs) work on quality, financial, and performance improvement activities, as well as enhance emergency medical service (EMS) and access to care. 
  • Rural Health Outreach Services Programs 
    • S. 2301/H.R. 2493, the Improving Care in Rural America Reauthorization Act reauthorizes Outreach programs which expire at the end of 2025. The umbrella of Outreach programs improve rural health through community-driven initiatives focused on quality improvement, health care access, and coordination of care to foster sustainable solutions for chronic disease prevention and management in rural areas. 

The following need an initial authorization in statute: 

  • Rural Residency Planning and Development Program (RRPD) 
    • The H.R. 3890, the Resident Physician Shortage Reduction Act includes a provision to authorize the RRPD program. RRPD helps to expand the number of rural residency training programs in family medicine, internal medicine, general surgery, psychiatry, and obstetrics by providing startup funding to eligible entities. The pilot began in 2019 and such since shown strong outcomes by creating 54 new, accredited rural residency programs and 683 resident positions. 
  • Rural Communities Opioid Response Program (RCORP) 
    • The RCORP Authorization Act (H.R. 9842 in the 118th Congress) authorizes RCORP to reduce the factors that result in substance use disorder and opioid use disorder morbidity and mortality in high-need rural communities through rural-specific prevention, treatment, recovery, and workforce development efforts. 
  • Centers for Disease Control and Prevention (CDC) Office of Rural Public Health (ORH) 
    • S. 403/H.R. 3102, the Rural Health Focus Act, authorize the CDC ORH to serve as a focal point for rural public health and to implement a strategy to meet the unique public health challenges of rural populations across CDC programs and data.

OTHER LRHA ADVOCACY ACTIVITIES


  • January 2025 | LRHA provided information and feedback from our rural providers to inform the NRHA response to the OIG Report titled "Medicare Could Save Billions With Comparable Access for Enrollees if Critical Access Hospital Payments for Swing-Bed Services Were Similar to Those of the Fee-For-Service Prospective Payment System"
  • January 2025 | LRHA submitted letters to elected officials urging them to help maintain federal funding, without interruption, for core rural health programs during any review and/or freeze of federal funding by the incoming Administration.
  • February 2025 | LRHA led Louisiana rural health leaders in meetings with federal legislative offices during NRHA Rural Health Policy Institute. LRHA then provided additional follow up information on the needs and challenges of rural providers to all offices, including providing additional information on high-priority needs to Speaker Mike Johnson's office.
  • February 2025 | LRHA contacted federal lawmakers to urge the protection of Medicaid funding.
  • March 2025 | LRHA began meeting with Louisiana rural health stakeholders to prepare for state legislative session.
  • April 2025 | LRHA contacted federal lawmakers to urge the protection of rural health program funding in response to proposed HHS budget.
  • May 2025 | LRHA conducted a listening session and submitted a comment letter in response to the Office of Management and Budget's Request for Information: Deregulation.
  • May 2025LRHA Executive Director co-authored a NRHA Member Perspectives blog post regarding the State Office of Rural Health Program.
  • June 2025 | LRHA submitted a response to the CMS Request for Information on Regulatory Relief
  • June 2025 | LRHA submitted letters to Louisiana state House Health & Welfare Committee regarding SB130 and SB96.
  • June 2025 | LRHA continued advocacy efforts around funding for Medicaid, including meeting with Senator Cassidy's office and responding to a Request for Information from Senator Sanders.
  • July 2025LRHA is reviewing and assessing the potential impacts of the One Big Beautiful Bill Act.
  • July 2025 | LRHA continued advocacy efforts around Medicaid cuts, including facilitating a rural hospital Administrator media interview.
  • July 2025 | LRHA joined the reconvening of the Health Disparities in Rural Areas Task Force on July 31, 2025.
  • July 2025 | LRHA finalized preparations for the Louisiana Rural Health Policy Forum on August 6, 2025.
