COMMUNITY CONNECTIONS
Partnership with Community HealthWays
Together, we’re increasing the use of community health workers (CHWs) in EMG-served parishes. By partnering with the Louisiana Office of Public Health's Community HealthWays program, we are able to offer this program and their services to the our EatMoveGrow participating schools. Through this partnership we will work collaboratively to provide information and resources to the parents and families of our schools.
EatMoveGrow is committed to providing CHW services to all 22 parishes and schools, bridging critical health gaps. The CHW model reduces rural-urban health disparities, improves individuals' quality of life, and boosts community engagement in preventative health activities.
A CHW is a trusted community member with an close understanding of the community served and its available resources. The Community Health Worker assists individuals in addressing social problems that lead to poor health, including food insecurity, housing, transportation, utility assistance, furthering education, mental health connections, and education related to chronic conditions.
The Community HealthWays program can help connect individuals with needed resources. Louisiana Rural Health Association (LRHA) is leading a pilot, Clinic Connections, in certain EatMoveGrow Parishes based on the evidence-based healthcare exchange model. This model links a rural health clinic with an EMG school, promoting collaboration among healthcare professionals, school administrators, and community leaders to enhance healthcare access.
The Clinic Connections Project is a trial initiative that is bringing the proven benefits of evidence-based health exchange models from urban to rural landscapes. Our trial project plans to impact the rural areas served by this project, by creating formal connections between EMG schools, local rural health clinics, and the dedicated state-funded community health workers assigned to each parish.
Within the framework of the Clinic Connections Project, our local teams are identifying the unique needs of schools and developing strategic plans to connect school communities with essential health resources to meet these needs. This project aims to provide more resources for rural students and their families and also make the community more aware of existing health resources that are underutilized.

August 7, 2024 Community HealthWays and EatMoveGrow Community Collaborations representatives attended Port Barre Elementary School's Teachers Professional Development Day to introduce the Community HealthWays program and how the community health workers could work the school by provide resources, information, and assistance to the families of St. Landry Parish. |
August 22, 2024 Community HealthWays and EatMoveGrow Community Collaborations representatives attended Rosenwald Elementary School's Back to School event where they shared information about their partnership and the assistance their programs could offer the school and families of Pointe Coupee Parish. |
September 17 & 18, 2024 The EatMoveGrow Community Collaborations partnership in Assumption Parish hosted a two day vaccination event for the families of Assumption Parish. Representatives from Napoleonville Primary School, Reddy Family Medical Clinic, Community HealthWays, and the Office of Public Health worked together to plan this event for their community. |
October 23, 2024 The EatMoveGrow Community Collaborations program shared information about their partnership opportunities with those in attendance at the annual EatMoveGrow Teacher Summit in Baton Rouge. Kathy Williams, Region 2 Community HealthWays Supervisor, provided information on ways the community health workers could work with the schools to provided assistance to their families and community. |
February 22, 2025 Rosenwald Elementary School staff completed the Youth Mental Health First Aid training that assisted in providing insight and knowledge for them to better assist their students during a mental health challenge. |
April 10, 2025 Golden Meadow Elementary School as part of their partnership through the EatMoveGrow Community Collaborations program will be providing their 3rd, 4th, and 5th grade students with tips and tricks for helping to manage anxiety through interactive mini lessons. The partners thought this would be a great opportunity to provide the students with invaluable information and techniques that can assist them in managing any anxieties they may experience prior to state testing. |
